Social Determinants of Health (SDoH) are non-medical factors that influence health outcomes and have influence on health inequities. Our Social Determinants of Health Program is designed to assist the most vulnerable members of society with the intent to improve behavioral health outcomes through the barriers impacting their health. Services include, but are not limited to social determinants of health surrounding housing, transportation, food insecurity, and childcare.
Must currently receive Medicaid health insurance with a managed care component in the County they are applying in.
Bedford, Blair, Fulton, Huntingdon, Juniata, and Mifflin County.
